Purpose
We propose integrating the development of decision analytic (DA) models into the process organized by the ACCP for guideline development and dissemination of antithrombotic therapy in patients with atrial fibrillation (AF).
Specific aim 1: To develop decision analytic models to examine multiple antithrombotic therapies for patients with atrial fibrillation, including novel oral agents that have yet to receive FDA approval. The models will facilitate the incorporation of individual patient preferences for health states, such as life following stroke with major or minor neurological sequelae, so that patient values and preferences can be explicitly considered in the decision-making process.
Specific aim 2: To use the model development and analysis process to help inform the guideline and recommendation generation process for antithrombotic therapy in patients with atrial fibrillation for the ACCP’s 10th edition of Antithrombotic and Thrombolytic Therapy.
Specific aim 3: To develop and evaluate tools that will enhance these guidelines to facilitate the incorporation of individual patient values and preferences for treatments and health outcomes through a shared decision making process with clinicians. Patient interviews and focus groups will be used to advance this aim.
Specific aim 4: To assess whether this process was found to be helpful by members of the guideline panel, and whether results of the decision analytic models actually were used to generate guideline recommendations.
