Primary Care Demonstration Sites
The Foundation is currently funding 11 primary care sites that will contribute to our understanding of how to integrate shared decision-making and decision aids, developed by the Foundation and produced by Health Dialog, into day-to-day clinical practice. We feel these primary care sites have the potential to have a significant, positive impact on engaging patients as partners with their health care providers when decisions are made about medical tests or treatment.
